Op-Walk USA Day Dec 3rd, 2011, WOGO Team Florida, Dr. Jennifer Cook
*** This Program will provide FREE Knee Replacements for up to 3 Patients who must be Pasco County Residents ONLY between the ages of 18 to 65 and qualify for the Program.
Qualification for Prospective Patient(s):
Dr. Jennifer Cook, of WOGO Florida, a Team of Operation Walk, will be performing free Knee Replacements for up to 3 Patients in need that qualify for the Op-Walk USA DAY Program. To see if you qualify for the Program you need to:
- Be a Resident of Pasco County
- Must be of age 18 to age 65
- Have mobility problems due to your knee(s)
- Need a Knee Replacement due to your condition
- Have No Insurance and No ability to pay for your procedure
- Be height to weight proportionate
- Be a Non-Smoker
- Meet all additional health requirements & screenings during the process
To begin the qualification process for this program you must first to set up an appointment to be Pre Screened by the Good Samaritan Clinic of Pasco County, New Port Richey, to find out if you can be selected for the program based on your medical condition and financial situation.
ONLY (up to) 3 Patients, will be selected to receive FREE Knee Replacements.
Once selected all of your pre care and surgical care will be provided to you at no charge, and a limited amount of post surgical care will also be provided for you through the program and it’s benefactors. Full details will be provided to you.
To Apply for this program:
All applicants must first contact the Good Samaritan Clinic of Pasco County by Phone at (727) 848-7789 to make an Appointment for a pre screening. Web: www.http://goodsamclinic.org. Once you have made an appointment you can download the required forms and bring them filled out with you for your appointment.
The Deadline for inclusion in this program is Tuesday November 8th 2011, and your Surgery Day will be December 3rd, 2011.
About Dr. Jennifer Cook:
Dr. Cook is a Board Certified Orthopaedic Surgeon and a member of the Arthroscopy Association of North America. She is from Great Falls, Virginia, where she graduated as Valedictorian in a class of over 400 students. She then attended the University of Michigan where she graduated Summa Cum Laude with a degree in Mechanical Engineering. After Graduating from Michigan, Dr. Cook attended the Health Sciences and Technology Program of Harvard and the Massachusetts Institute of Technology, earning a Medical Doctorate Degree from both Harvard and MIT. She completed her Orthopaedic Residency at the Harvard Combined Orthopaedic Program where she was appointed Chief Resident. From there, she went on to complete the prestigious Insall Scott Kelly fellowship in adult reconstruction and sports medicine in New York City. She is also one of the founding Doctors of WOGO, and Team Leader for WOGO Florida.
